A License to Save Lives Download Form
Please fill out the basic information below in order to download Gift of Life's high school education program.
A License to Save Lives is a new program created by Gift of Life Donor Program in collaboration with Gift of Life Institute. This program is designed to supplement educational programs for high school students that teachers, students, and volunteers alike can all use.
Gift of Life will award up to 10 hours of community service for students who complete a survey after watching A License to Save Lives. To get started, fill out this form to download the program. For more information, contact the Gift of Life community outreach team at communityoutreach@donors1.org.
