Donate Life License Plates
Show your support for donation by getting a Donate Life License Plate. Residents in Pennsylvania, New Jersey and Delaware can order a plate.
Pennsylvania Donate Life License Plate
Plates cost just $24 and are available for passenger cars, motor homes and trucks with a registered gross weight of 9,000 pounds or less.
To apply, simply print a license plate order form and mail with your $24 check or money order ($20 for the plate and $4 for the Department of Corrections production fee) payable to:

To purchase additional registrations, add $24 per registration to your payment. The Division of Motor Vehicles will not accept any Donor Awareness License Plate applications that are not forwarded by the Transplant Foundation. New Jersey Donate Life License Plate
The Organ Donor license plate benefits the Organ and Tissue Donor Awareness Education Fund, which educates the public about becoming an organ and tissue donor. Funds will also be distributed to the organ procurement organizations serving New Jersey to increase public awareness and education about donation and transplantation in the Garden State.
To get your New Jersey plates, just go to any motor vehicle agency with your current plates and registration and pick up the new Organ Donor plates, or call 888-486-3339 (toll free in New Jersey) on weekdays from 8:30 a.m. to 4:30 p.m. to order by mail. From out of state call 609-292-6500. Plates cost $50 with an annual $10 renewal fee. You can order personalized plates with up to five characters for $100 and a $10 yearly renewal by mail.

Delaware Donate Life License Plate
In partnership with the state's Department of Transportation, Delaware residents can now purchase a Donate Life license plate for their car.
